What is Project H.E.A.L?

  • A community outreach program from BRIDGE that helps older adults in Brooklyn learn about healthy aging.

  • Focuses on preventing sickness, disability, and injuries, and supports you in staying healthy and active.

Project H.E.A.L’s Mission:

  • Provide easy-to-understand health information for older adults in Brooklyn.

  • Offer tips and strategies to help everyone age healthily—no matter your income, background, or abilities.

  • Work with local partners (like senior centers and health groups) to address challenges that might prevent older adults from living their best lives.

Key Activities:

1

HEALTHY AGING TALKS

Bi-weekly healthy aging education with our community partners. Our talks include a broad range of topics related to our 4M Framework (i.e., What Matters, Medication, Mentation, and Mobility)

2

4M's CHAMPIONS PROGRAM

We propose to educate and train a cohort of seniors (4M Champions) at each center on how to promote age and dementia-friendly care.

3

INTERGENERATIONAL
LEARNING COLLABORATIVE

A special program bringing together older adults and medical students from SUNY Downstate to foster understanding between generations, share knowledge, and address age-related stereotypes.
